Congratulations on taking the first step toward helping eliminate some of the hardships your family may experience at the time of your death. By pre-planning your own services, you are lifting the burden from your loved ones and leaving a legacy that your family will appreciate.

A casket selection is a very personal decision and an expression of your feelings. A casket that reflects your personality and taste can be the final tribute to your life. We understand these needs and work with you to offer a wide variety of caskets that can help make your selection truly personal.
A burial vault is a lined and sealed outer receptacle that houses the casket. It protects the casket from the weight of the earth and heavy maintenance equipment that will pass over the grave. It also helps resist water and preserves the beauty of the cemetery or memorial park by preventing the ground from settling.
The Deceased Information
The following information is used to complete the legal certificate of death in Virginia. To insure identity protection, we only ask for certain information. Additional information will be obtained when you come in for the arrangement conference, such as, Date of Birth, Social Security Number, and Mother's Maiden Name of the deceased.
*Due to security reasons we will obtain the deceased's mother's maiden name at the arrangement conference.
Family Information
Please list the names of survivors and state their relationship to you, their spouse's names and the city in which they live as you wish to have them listed in the obituary.
(Example: Nancy Smith, Sister, Richmond, VA)
(Immediate family members you wish to list in obituary)
(Please note: "Retired" is not an acceptable occupation)
Additional Submitter Information
 

To help us better serve you, may we suggest you bring the following items and information when you arrive at the funeral home for your appointment:

  • Deceased's Mother's Maiden Name
  • Clothing, including underclothing. If desired, we have a selection of clothing available, which may be purchased.
  • Eyeglasses and jewelry, which can be removed and returned to the family following the services.
  • A recent photograph of the deceased
  • Veteran discharge papers, commonly called the DD-214.
  • Social Security card
  • Life insurance policies
